Collapse "create new thread" and "post comment" buttons into one :implemented:

Right now if you want to post a comment on a chapter that has no comment thread yet, you need to:
1) press "Comment" button under the last page of the chapter;
2) press "Start comment thread" on the page that opens;
3) press "Comment" once the (now empty) thread has been created;
4) actually write the comment and submit it.

I understand that the comment system will be reworked in foreseeable future, and right now there are placeholders and crude temporary solutions, but right now two things could be improved relatively quickly (?):
1) actions #1 to #3 could be collapsed into one or at most two actions (at the very least #2 and #3 could be executed with one button press);
2) empty chapter comment threads could be created automatically upon chapter submission until the moment we can post more than one thread per chapter.

These seem like sensible solutions but I'll have to wait for Holo to look in and see what he thinks. Marking as Maybe for now.
