In their previous lives, NC was in love with a girl, but they can't marry, so they're eloping, when the boy dies (possibly by drowning).

MC is reincarnated in present day, and realises that his faithful servant has also reincarnated, as a girl who is his classmate, and they both regain their memories. Then he tries to find the reincarnation of his lover, but he finds out she's his sister in this life. After that he and the girl (reincarnated servant) get together.

There was also a fortune teller involved, the reincarnated servant calls MC 'Bocchan', and talks to him like an old servant would, rather than a teenage girl, and always tries to take care of him, and I think the MC's sister and his best friend start going out. (Also his best friend might have been a reincarnation as well.)
